AI governance challenges: How to support safe, scalable AI adoption

AI governance is essential for helping enterprises adopt AI safely, consistently, and at scale.

But as AI use expands across a business, mismatches can appear between the organization’s governance framework and how teams actually use AI.

In this article, we explore common AI governance challenges and failure modes, and outline steps enterprises can take to address them. Why AI governance gets harder as adoption scales AI governance is easier to manage when AI use is limited to a small number of controlled pilots. At that stage, the organization knows which teams are involved, what data is being used, what the intended use case is, and who is responsible for review.

Oversight becomes more complex when AI adoption expands beyond those controlled settings. For example, a tool initially approved for a low-risk internal task may end up being applied to higher-stakes customer-facing work; a vendor product may add AI features after the enterprise’s initial procurement or security review; or employees may start using publicly available AI tools before the organization has set clear rules for acceptable use.

In these situations, the risk is not AI use itself. It is that the organization may lose visibility into where AI is being used, who is accountable for it, and whether the right controls are in place for each use case. Where AI governance problems show up in practice AI governance problems often arise when the rules, review processes, and ownership models that worked for early AI use are too narrow for broader adoption across the business.

Here are some examples of how these issues can show up. When ownership of a specific system or use case is not clearly assigned, each team may assume another group is responsible for key governance decisions, including whether the use case is appropriate, what data the system can access or process, how outputs should be reviewed, and who responds if something goes wrong.

For example, a business team may sponsor an AI use case, IT may manage access, security may review the vendor, and compliance may advise on policy. But if no one in particular owns the application after launch, issues such as shifting usage patterns, data exposure, and unreliable outputs can fly under the radar. Situations like this can create privacy, security, contractual, or compliance risk, even when the AI use case is useful and well-intentioned. How to address AI governance challenges Effective AI governance depends on adopting practical operating processes. The aim is to make governance specific enough to direct how teams review, deploy, and use AI systems, while remaining adaptive as those systems and usage patterns change.

Here are some practices enterprises can consider as they evaluate responsible AI governance as adoption expands.
